Medical Supplies Order (2023)

Welcome to the Alberta MFR supply replenishment form! 

Please note that the order form may time out after 20 minutes and a draft of your order will not be saved. To avoid timing out, stay on this screen until the order is submitted. Don't forget to complete the visitor confirmation code at the bottom of the order form to ensure a successful submission. To search for an item, press 'Ctrl - F' and type the name of the item.

In an effort to reduce shipping costs, please limit orders to once every 3-4 weeks whenever possible.

The items listed represent the most commonly-requested supplies. If the item you are looking for is not listed, it is not stocked at the EMS warehouse. Quantities may be limited for some items.

Please allow 2-3 weeks for delivery and ensure you provide a street address for courier delivery. Some departments choose to have orders delivered to the town office or to their place of employment to ensure someone is available to accept it. Once you have placed your order, you will receive automatic email confirmation if the order was successfully submitted.
